TEAM BUILDING   Before you start, raise awareness in your institution by getting a multi-disciplinary team of health care professionals and managers involved in the process. It will be important to take this opportunity to discuss the issues raised by chronic disease management for people with COPD and to agree upon shared goals.

Build a successful team

A successful team should include:

1. Health care managers
2. Health care providers (physicians, other health care professionals)
3. Information/medical records staff
4. Hospital Quality Department staff
5. Stakeholders

You should state the benefits to each of these participants and understand the context, mission, vision and values.

Do not assume that you know the impact of COPD on your institution before you have done a formal assessment.
